For planned absences we require you to fill in an absence request form, this then goes to the headteacher for authorisation. You can get an absence request form from reception, or you can download it below.

Please submit the request form into reception or email it to attendance@coombedean.co.uk.

Please complete form. Save the form as: "PARF-Addsurname"and attach to email. Send email to attendance@coombedean.co.uk.

You do not need to fill out an absence request form for medical or dental appointments, but please let us know in advance by contacting us either by phone or email  to attendance@coombedean.co.uk.
